Medical Statement for Diving
At Sunshine Dive Resort safety is a priority and prior to participating in any dive activities here, each participant must complete a medical form to ensure that their health is suitable for diving.
Please have a look at the Medical Questionnaire below and if there are any medical issues you have answered “YES” to then a medical form from a doctor is required before you can participate in any diving activies.
